I acknowledge that this is a public kiosk and the information I am about to submit should not be viewed by anyone other than myself. I will use caution while typing in confidential information (passwords, etc.) to ensure that no one else can see it.

I understand and accept the agreement and conditions of use for this kiosk.



CAUTION: Please note that this kiosk is programmed to return to the screen saver after 2 ½ minutes of inactivity (no data entry or screen touches). If this occurs while you are submitting information or completing an application, all entered information may be lost. To prevent this from happening, do not leave the kiosk computer idle for more than 2 ½ minutes. You may touch the screen periodically to keep it active.

You will not be able to return to the main menu until after the screen saver has been activated.
